Become a patient

Check with your medical insurance to see if we are providers. If not, you will need to be referred to us from your primary care physician (not your dentist, as we are covered under medical insurance, not dental insurance.)

We are usually booked a few months in advance but would be happy to put you on a wait list and call if something opens up sooner.

What to bring

We mail out a new patient packet. Please bring to your appointment:

  • Completed forms from the new patient packet you received in the mail
  • Photo ID
  • Medical insurance card
  • Parking ticket for validation

What to expect

At your first appointment, a dentist will do a thorough examination to determine your diagnosis and develop your treatment plan. Additional evaluation appointments with one of our physical therapists and/or health psychologists may also be recommended to TMD/Orofacial pain patients, to complete the evaluation process. Other testing (X-ray) and consultation appointments will be discussed, if necessary.

  • A basic treatment program for TMD/Orofacial pain conditions can involve three to six months of a supervised home program involving exercise, habit change, and monthly visits to the clinic.
  • Patients with more complex needs are generally seen at least weekly for four to eight weeks. Program involvement is then tapered as symptoms decrease.

Price and insurance information

We use medical insurance, not dental insurance. We accept most medical plans but not medical assistance.

Treatment costs vary. Out of pocket expenses are usually similar to those for other rehabilitation services.

Payment

These services, if covered, are a medical insurance benefit. Please check your individual medical insurance policy for specific plan coverage.

If you do not have insurance coverage, payment is expected at the time of service.
